Attach the following documents:
1. Fully accomplished Personal Data Sheet (PDS) with recent passport-size picture (CS Form No. 212, Revised 2017) which can be downloaded at www.csc.gov.ph with Work Experience Sheet (WES);
2. Photocopy of certificate of eligibility/rating/license, if applicable;
3. Photocopy of Transcript of Records and Diploma;
4. Photocopy of certificate of seminars/training, and
5. Valid/recent X-ray Report (to be submitted by the successful applicant before deployment)
