1. Fully accomplished personal data sheet ( PDS) with recent passport-sided picture ( CS Form No. 212. Revised 2017) which can be downloaded at www.csc.gov.ph with work experience Sheet (WES);
2. Photocopy of Certificate if eligibility/rating/license.
3 Photocopy of Transcript of Records and Diploma.
4. Photocopy of Certificate of seminars/trainings; and
5. Medical Certificate with latest x-ray report (to be submitted by the successful applicant before deployment)
